Stroke Management (Thrombolysis)

Stroke Management (Thrombolysis)

Thrombolysis is an emergency treatment that uses clot-dissolving medicine to restore blood flow during an eligible acute ischemic stroke. Rapid diagnosis and treatment are critical because earlier restoration of blood flow can reduce permanent brain injury and disability.

Overview

1. What is Stroke Thrombolysis?

Thrombolysis is a treatment for selected patients with an acute ischemic stroke, where a blood clot blocks blood flow to part of the brain. A thrombolytic medicine is administered to dissolve the clot and restore circulation.

2. When is Thrombolysis recommended?

It may be considered for eligible patients with an acute ischemic stroke who present within an appropriate treatment window and meet clinical and imaging criteria. The exact eligibility depends on the time last known well, stroke severity, brain imaging, medications, medical history, and bleeding risk.

3. Why is rapid treatment important?

Brain tissue can be permanently damaged when its blood supply is interrupted. Stroke is a medical emergency, and assessment and treatment should begin as quickly as possible rather than waiting for symptoms to improve.

4. How is Thrombolysis performed?

After urgent clinical assessment and brain imaging, an appropriate thrombolytic medicine may be administered intravenously. The patient is then closely monitored for neurological changes, blood pressure, bleeding, and other complications.

5. What tests are performed before Thrombolysis?

Urgent brain CT or MRI is used primarily to determine whether the stroke is caused by bleeding and to assess brain injury. Blood tests, blood-pressure assessment, neurological examination, and vascular imaging may also be required.

6. Can Thrombolysis be combined with Mechanical Thrombectomy?

Yes. Some patients with a large-vessel blockage may be eligible for mechanical thrombectomy, in which the clot is physically removed using an endovascular catheter. Thrombolysis may be given before thrombectomy when appropriate.

7. What are the benefits?

For eligible patients, timely thrombolysis can improve the chance of functional recovery by restoring blood flow and limiting the extent of brain damage. It does not guarantee recovery, and treatment effectiveness depends on several factors.

8. What are the risks?

The most important complication is serious bleeding, including bleeding within the brain. Other possible complications include bleeding elsewhere, allergic reactions, blood-pressure changes, and worsening neurological symptoms. Careful patient selection is therefore essential.

9. Why consider Stroke Management in India?

India has specialized stroke centres, experienced neurologists and neuro-interventional teams, emergency CT/MRI facilities, stroke units, ICU capabilities, and access to thrombolysis and mechanical thrombectomy where indicated.

10. How can Medavelle help?

For suspected acute stroke, emergency medical care should be sought immediately rather than arranging international travel through Medavelle. For patients already stabilized or seeking specialist review, Medavelle can help coordinate neurological evaluation, review existing medical records and imaging, identify an appropriate stroke centre, and arrange rehabilitation or further management in India.

FAQs

No. Thrombolysis is primarily used for selected ischemic strokes caused by a blood clot. It must not be given when the stroke is caused by bleeding in the brain.
